Zygomatic implants are a specialized solution for patients who have experienced significant bone loss in the upper jaw. Instead of relying on traditional implant placement, these implants anchor into the zygoma (cheekbone), providing a stable foundation without the need for extensive bone grafting.

The cheekbone is one of the densest, most reliable structures in the skull — and it rarely loses density the way the upper jaw does. By placing implants into this stable foundation, we can deliver same-day fixed teeth to patients who would otherwise require months of bone grafting, multiple surgeries, and extended healing periods before even being considered for traditional implants.

This procedure was specifically developed for patients who’ve been told they’re not candidates for implants — and at Advanced Implant Specialist, it’s one of our specialty procedures. Dr. Winfield’s training in advanced full-arch rehabilitation and complex implant techniques makes him one of the few specialists in San Diego performing this procedure routinely.

Most zygomatic cases are paired with All-on-X full-arch restoration, allowing patients to leave the same day with a complete, fixed smile.

The Solution

What Are Zygomatic Implants?

The Anatomy

How Zygomatic Implants Work.

Traditional dental implants are placed into the alveolar bone of the upper jaw. When this bone has resorbed (deteriorated), there’s not enough volume to support standard implants — which is why many patients are told they need bone grafting.

Zygomatic implants take a different anatomical path. They are longer than standard implants and angled upward to anchor into the dense bone of the cheekbone (zygoma) — bypassing the deteriorated upper jaw entirely.

Longer implants

Specially engineered length to reach the cheekbone

Strategic angle

Placed at a specific angle to maximize bone-to-implant contact

Cheekbone anchorage

The zygoma is dense and resistant to resorption

Immediate stability

Sufficient stability for immediate-load same-day teeth

The Gold Standard

Why Patients Choose Zygomatic Implants

For patients with severe bone loss, zygomatic implants deliver outcomes traditional approaches simply can’t.

No Bone Grafting

Avoids sinus lifts and bone grafting in many cases — saving you months of healing, additional surgeries, and significant cost.

Immediate Function

Same-day teeth means you walk out with a fixed, functional smile — no months of waiting with gaps or temporary dentures.

Hope Where There Wasn't

Ideal for patients previously told they are not candidates for traditional implants — the procedure was developed specifically for these cases.

Faster Treatment Timeline

Skip the 6–12 months of bone graft healing — go directly from consultation to placement to permanent teeth.

Highly Stable & Predictable

The cheekbone is one of the densest structures in the skull — providing exceptional stability and predictable outcomes.

Premium Final Restorations

Final teeth are crafted from medical-grade zirconia — engineered to look natural and last decades.

Candidacy

Who Is a Candidate?

If you’ve been told you don’t have enough bone for dental implants, you may still be a candidate for zygomatic implants. The procedure is specifically designed for patients who fall outside the criteria for traditional implant placement.

“Many patients come to us after being told ‘no’ by other providers. Most of those patients become candidates for zygomatic implants — and walk out with same-day fixed teeth.”

The only way to know definitively is a free 3D consultation with comprehensive CBCT imaging. We evaluate your bone density, sinus position, and overall health to determine the best approach for your specific case.

Why a Specialist Matters

Performed by a Board-Certified Specialist.

Zygomatic implants require advanced training and experience. The procedure involves the cheekbone, proximity to the maxillary sinus, and complex angulation — requiring expertise that goes far beyond standard implant training.

At Advanced Implant Specialist, your procedure is performed by a board-certified prosthodontist specializing in complex full-arch rehabilitation and advanced implant techniques. Most general dentists do not perform zygomatic procedures — and even most implant specialists refer these cases out.

Dr. Winfield’s training in prosthodontics and complex restorative care makes Advanced Implant Specialist one of the few practices in San Diego performing zygomatic implants regularly and confidently.

Patient Story

They said it wasn't possible..

“Three providers told me I’d need a year of bone grafting before I could even think about implants. Dr. Winfield placed zygomatic implants in one day. I had teeth that evening.”

FAQs

Everything you need to know about zygomatic dental implants.

What are zygomatic implants?
Zygomatic implants are specialized dental implants that anchor into the zygoma (cheekbone) instead of the upper jaw. They are used for patients with severe upper jaw bone loss who would otherwise require extensive bone grafting before traditional implants could be placed. The cheekbone is one of the densest bones in the skull and rarely loses density — making it an exceptional foundation for implants.
Yes — zygomatic implants are specifically designed for patients told they have insufficient bone for traditional implants. Because they anchor in the cheekbone (which rarely loses density), zygomatic implants offer a solution when other approaches fail. Even after multiple “no” answers from other providers, most patients become candidates for zygomatic implants.
In most cases, no. The procedure was specifically developed to eliminate the need for bone grafting and sinus lifts in patients with severe upper jaw bone loss — saving months of healing time and avoiding additional surgeries. Some complex cases may benefit from a combination approach, but the majority can be done without grafting.
Yes. Zygomatic implants are typically placed using an immediate-load protocol, meaning patients can leave the same day with a fixed, functional smile — just like our All-on-X procedure. Most patients return to a normal soft-food diet within hours of leaving the office.
Zygomatic implant placement involves the cheekbone and proximity to the maxillary sinus — requiring advanced surgical training, 3D planning expertise, and complex case management. Most general dentists do not perform zygomatic procedures, and even most implant specialists refer these cases out. At Advanced Implant Specialist, the procedure is performed by a board-certified prosthodontist with advanced training in complex full-arch rehabilitation.
With IV sedation, patients experience minimal to no discomfort during the procedure. Most describe sleeping through it. Post-surgical soreness is managed with prescribed medication and typically resolves within 5–7 days. Most patients return to work within a week of surgery.
